When it comes to achieving fuller, more defined lips, there are two popular options: Lip Filler and the Lip Flip. While both treatments can enhance lips, they work in different ways and are suited for different aesthetic goals. In this article, we break down the key differences between lip filler and lip flip, so you can determine which treatment is right for you.
Lip Flip vs Lip Filler
A lip flip procedure utilizes Botox, while lip filler involves a dermal filler like hyaluronic acid (e.g., Juvederm). Both can enhance the appearance of your lips, but they differ significantly. The lip flip offers a subtle fullness without adding volume, lasting about three months, whereas lip filler provides a more pronounced enhancement and can last up to a year by adding volume and smoothing out lip lines.
The application methods also vary. A lip flip is a quick, minimally invasive treatment where Botox is injected into the upper lip line, causing a gentle upward flip that can help reduce the visibility of gums when smiling. In contrast, lip filler requires more extensive injections across the lips, often accompanied by a numbing cream, though some discomfort, swelling, and bruising may occur afterward. Deciding between the two depends on your desired outcome and comfort level with the procedures.
How Lip Filler Works
Dermal fillers are broadly classified into two primary types: hyaluronic acid fillers, which are frequently used for lip enhancements, and biostimulators. Many doctors prefer hyaluronic acid fillers due to their temporary nature, reversibility, and the fact that they complement the natural hyaluronic acid present in the body. New York City-based board-certified dermatologist Macrene Alexiades M.D, explains, “When hyaluronic acid fillers are injected into the lips, they immediately work to attract water, which in turn, adds volume to the region where it was injected to give a fuller appearance.” ¹
Common Hyaluronic Acid (HA) Fillers
The two most popular types of HA Fillers used for lip augmentation are Juvéderm and Restylane. Juvéderm contains a highly cross-linked form of hyaluronic acid and is not broken down very quickly. Results with Juvéderm are typically immediate and long-lasting, around nine to twelve months before additional treatment is required.
Restylane is another family of hyaluronic acid lip fillers that creates volume. Designed to offer a natural lip filler, Restylane enhances lip volume and smooths lip lines or wrinkles around the mouth. Results with Restylane will be noticeable a few days after the procedure and can typically last six to nine months.
Lip Filler Steps
- Consultation. The lip filler process starts with an assessment of your lip anatomy and your aesthetic goals. Filler options will be discussed, and you and your GMS practitioner will create a treatment plan.
- Numbing. Pre procedure, a topical anesthetic or local injection will be applied to numb your lips to minimize discomfort. This usually takes 15–30 minutes.
- Injection. Once the numbing is in effect, your practitioner will use a thin needle or cannula to inject the filler into specific areas of your lips. They may target the edges of your lips, the curve in the center of your upper lip, or the corners of your mouth.
- Massage. After the injection, your practitioner may massage the treated area to help distribute the filler evenly.
- Post-care. You will receive instructions on how to care for your lips after the procedure. This may include avoiding strenuous activities and too much sun or heat exposure.
What is a Lip Flip?
As mentioned, a lip flip uses Botox to create the appearance of a plump, full lip. Botox is injected into the muscles around the upper lip, causing the lip to relax and gently flip outward, creating the appearance of a fuller upper lip without adding volume. Lip flips are more effective at achieving symmetry and exposing more of the lip than hyaluronic acid lip fillers, but they don’t provide nearly as much volume. Results are more subtle than lip fillers and typically last around 2–3 months.
Key Differences Between Lip Filler & Lip Flip
Lip Filler:
- Dermal filler is injected
- Can last up to a year
- Customizable volume increase of the lips
- Adds actual volume to the lips using dermal fillers.
- Can reshape and contour the lips, making them fuller and more defined.
- Results are immediate and longer-lasting (6–12 months).
- Ideal for those looking for a dramatic enhancement.
Lip Flip:
- Botox is injected
- Lasts 8-12 weeks
- Gives you control over your lip shape
- Relaxes the upper lip muscles, creating a slight flip that gives the appearance of fuller lips without adding volume.
- Results are more subtle and natural-looking.
- No volume is added, so the lips won’t be significantly larger.
- Ideal for those looking for a subtle enhancement or to improve a gummy smile.

Pros & Cons of Lip Filler vs. Lip Flip
Lip Filler Pros:
- Instant, dramatic results.
- Customizable shape and volume.
- Longer-lasting than a lip flip.
Lip Filler Cons:
- Temporary Results: While longer-lasting than a lip flip, lip fillers are still temporary and need to be maintained every 6–12 months.
- Risk of Migration: Filler can sometimes migrate outside the intended area, causing unevenness or puffiness.
Lip Flip Pros:
- Quick, subtle enhancement.
- Minimal downtime, with less swelling or bruising.
- Less costly than filler.
Lip Flip Cons:
- Results are subtle and temporary (lasts only 2–3 months).
- Doesn’t add volume, so those wanting fuller lips may still opt for filler.
Combining Lip Filler & Lip Flip
Both lip filler and lip flip treatments can be combined for enhanced results. For example, some patients choose to start with a lip flip before deciding to move on to lip filler. Or patients may opt for a dermal filler in the lower lip and a lip flip above. Lip flips may also be added to lip fillers injected a few months later to enhance the shape and extend the results.
